STATEMENT OF THE JOB


The Family Assistant is responsible to maintain the health records of children enrolled in the program. Responsibilities include but are not limited to supervise and support Social Service staff in the absence of the Family Service Coordinator (FSC), maintain children’s files in accordance to ACS/DOHMH regulations and compliance standards.  
 

DUTIES AND RESPONSIBILITIES


  • Responsible for imputing information in the data master in collaboration with the Social Service staff.
  • Develops and implements Family Partnership Agreements with Families and provides ongoing support to the Family Worker (s) in these efforts.
  • Responsible to ensure that all children’s immunizations are current; arranges hearing, vision, and dental screening for all children.
  • Responsible to follow up with illness and allergies of children; supplies list of allergies to all services staff and classroom staff.
  • Ensures that care is provided to children who arrive to school ill or become ill during the school time; notifies parents of children’s illness.
  • Follows up with Education Staff to ensure that notice of observation are received regarding any child who comes to school with an injury.
  • Plans and coordinates Health Education Workshops and Health Fairs for parents with the Family Service Coordinator and Family Worker educational trainings for parents utilizing the community resources.
  • Maintains children’s files in an organized manner by checking out systems for all records.
  • Compiles and updates files for an individual child and follows up with any missing information to provide ongoing support to Family Workers.
  • Develops and implements Family Partnership Agreements with Families and provides ongoing support to the Family Worker (s) in these efforts.
  • Coordinates Parent Involvement Program (s), conducts home visits for all children in collaboration with Social Service and Education Staff, and facilitates team meetings with assigned classrooms. 
  • Adheres to all funding source standards in addition to the Agency (Catholic Charities Brooklyn and Queens) policies and procedures; seeks guidance/clarification from their immediate supervisor regarding interpretation, applicability, and implementation.
  • Strictly adheres to and enforces all the Agency policies regarding Confidentiality, HIPAA requirements, and Corporate Compliance regulations.
  • Supervises the Social Service staff in the absence of the Family Service Coordinator and the Education Director.
  • Participates in education staff meetings as liaison to Social Service staff in the absence of the Family Service Coordinator.
  • Assists in client recruitment efforts and registration in accordance with the Agency’s policies and Collective Bargaining Agreements.
  • Administers first aid to children if necessary.
  • Covers classrooms as needed..


SPECIFICATIONS FOR EDUCATION/LICENSES/CERTIFICATIONS


  • High School Diploma required. 
  • College Degree preferred.
  • Family Development Credential preferred.


SPECIFICATIONS FOR EXPERIENCE AND TRAINING


  • 2 years of experience in Human Services required.
  • Proficient in Microsoft Suit; Word, Excel, and Outlook.
  • Ability to communicate in a positive manner with management, staff, and parents.
  • Bilingual English/Spanish or English/Polish speaking preferred.

 

SPECIFICATIONS FOR PHYSICAL REQUIREMENTS

  •  stooping, kneeling, crouching, & crawling.
  • Occasional running.
  • Able to travel for inter-borough training, meetings, and escorting parents to meetings.
  • Must be able to occasionally lift and/or move up to 30 pounds.
